ASSISTED LIVING FAQ

Assisted living is ideal for people who can still live independently but need extra assistance with activities of daily living such as taking medication, bathing, dressing, making meals, housekeeping, transportation and other everyday needs. Our trained, caring staff members provide individualized support designed to help each resident maintain their highest level of independence and we have a nurse on staff 24/7.

In general, it’s best to check with your family doctor or other physician. When help with daily tasks is needed — taking a bath, getting dressed, using the bathroom, taking medication — assisted living services can be the answer. Our trained professionals provide the helping hand that preserves an individual’s independence.

MEMORY CARE FAQ

Memory loss, whether from Alzheimer’s or other forms of dementia, affects more than just one person. At Santa Marta, we work with individuals, family members and caregivers, searching for the right balance of respect and care.

  • Watch for behavior or lifestyle changes, and when help is clearly needed, go with your loved one for a professional evaluation.

Your loved one’s private suite is set in our safe and secure memory care neighborhood. Rooms are equipped with an emergency call system and monitored 24 hours a day. Indoor spaces are specially designed to reduce confusion and wandering. Your loved one can safely enjoy the outdoors in our secure courtyard and gardens.

They have the gift of being person-centered in their interactions and bring exceptional patience, gentleness and a passion for consistently providing dignified and respectful care to our memory care residents and their families. SKILLED NURSING FAQ

Skilled nursing is expert professional care rendered around-the-clock. It’s provided by licensed nurses and therapists and can be short term to help with rehabilitation or recovery, or long term to treat a chronic illness.

A person may need round-the-clock care after an illness, injury or surgery, but without hospitalization. They may also need a higher level of assistance with daily living tasks such as bathing, grooming and getting around. Some health conditions that could require skilled nursing include:

  • Stroke or cardiac illness
  • Congestive heart failure
  • COPD (chronic obstructive pulmonary disease)
  • Parkinson’s
  • Diabetes
  • Alzheimer’s or other forms of cognitive decline
  • Fractures or fall-related injuries
  • Orthopedic surgery
  • Other major illness or permanent disability

You can improve the effectiveness of the treatment plan when you prepare the right information before admission, including:

  • Health insurance, with policy numbers of private medical insurance, prescription plan, Medicare/Medicaid, long-term care insurance, plus vision and dental (if applicable).
  • Medical information, with doctor and pharmacy contact, medical diagnoses, allergies and drug sensitivities, health history, and a health event log with hospitalizations, surgeries and major health events (if any).
  • Medications, including (for each) the name, dosage, how and when to take it, why it was prescribed, start and stop dates, whether monitoring is required, the doctor who prescribed it and if there are any side effects.
  • If you hold the patient’s Durable Power of Attorney for Health Care, bring a copy. If you have a copy of their advance medical directives, such as a DNR (Do Not Resuscitate) order or a POLST (Physician Orders for Life-Sustaining Treatment) form, please bring it.

REHABILITATION FAQ

Our therapists will personalize a plan for your needs to help you move better, reduce pain, restore muscular function and prevent future injury. Physical therapy may include strength and mobility training, muscle reeducation and therapeutic exercise. Occupational therapy focuses on fine motor skills and improves your ability to perform daily tasks such as buttoning a shirt or writing. Speech therapy addresses changes in communication, eating and swallowing skills.

You will receive an individualized treatment plan based on medical certification by your doctor. We’ll care for you, guide your rehabilitation process, and work in partnership with you and your family members.

Before you’re discharged, we’ll review your treatment plan to determine if you’ll need in-home services or equipment. Santa Marta staff will be available to help with your transition home, providing therapy as needed. You may also visit Santa Marta to continue outpatient therapy.

RESPITE CARE FAQ

Respite stays vary from a few days to a few weeks but usually last less than a month.

Their personal and medical needs will be assessed, and with your help, we’ll create a care plan that incorporates their routines and preferences.

Absolutely. Family members and friends are welcome at any time.

We offer a full calendar of activities, programs and events every day. You and your loved one can decide which ways to make friends, stay active, and have fun are right for them.
